We require a pre-travel assessment form and a vaccination record to be completed prior to the consult so that the pharmacist may review information prior and look at the travel itinerary.

Fill out the following pre-travel assessment form and email it to our store.
We are a registered Yellow Fever Vaccination Centre through Health Canada and regularly provide international certifications for Yellow Fever vaccine or exemption certificates for those with contraindications. We keep the majority of requested travel related vaccines on hand in a professional grade vaccine refrigerator, and will liaise with practitioners for prescriptions required for Travellers’ Diarrhea, malarial chemoprophylaxis and altitude sickness.
